DIABETIC FOOTWEAR
If you are diabetic and covered by Medicare, you may qualify for partial reimbursement on approved shoes and inserts. Medical professionals agree that appropriate, properly fitting footwear can prevent many diabetes-related complications, including amputations.
HOW DOES IT WORK?
Medicare requires certain paperwork to be provided at the time of service for your diabetic shoes. Contact our store to provide your basic information that will help us gather the necessary paperwork required to receive your reimbursement.
REQUIRED PAPERWORK INCLUDES:
A prescription from your primary care physician (M.D. or D.O.) that includes:
Diabetic shoes
3 pairs of heat-molded diabetic inserts OR 3 pairs of custom-made diabetic inserts
CMN (Certification of Medical Necessity)
Medical history report that documents proof of:
Diabetes
A foot exam completed within the last 6 months qualifying your condition
